Need to Facilitate Advance Care Planning Discussions?
First Steps® Advance Care Planning (ACP) is the foundation for weaving ACP discussions into a routine of good patient care and preventive healthcare for all adults. When integrated in the outpatient or community setting, and early in the course of healthcare, First Steps discussions help to normalize the concept of planning, and orient individuals to the importance of regular review and update of written plans.

First Steps ACP Facilitator Certification has two parts.
contains three steps:

  1. Complete Respecting Choices First Steps® ACP Facilitator Online Core Curriculum.
  2. Complete “practice” Advance Directive document; submit to Instructor for review (this will be returned to participant). The Making Choices Michigan Advance Directive document is available at Scroll to bottom of page for a downloadable form in English or Spanish.
  3. Attempt to have an ACP conversation with a loved one or close friend. You will be asked to share what you learned from this personal exercise.

PART II is a one-day onsite session. To become certified as a RC ACP facilitator, you must complete both parts and the required assignments (complete the written advance directive and engage in an advance care planning discussion with someone close to you).

After you register for the course, you will receive an email with instructions for accessing the online modules, requirements for attending the onsite course, and information regarding completion of the assignments. You will receive a certificate of attendance after you complete all requirements. Please register at least one month before attending the onsite course so you have enough time to complete the online modules.

The purpose of this program is to assist participants in learning effective communication and interview skills for patient-centered ACP facilitation.
